Please read the following information carefully. By ticking the box below, you confirm that you are the legal next of kin or a person with the proper authority to make these arrangements, and you authorise MAS Logistics (UK) Ltd to act on your behalf.

Client Authorisation: I, the applicant, hereby instruct MAS Logistics (UK) Ltd of Unit 15 Barratt Industrial Park, Park Ave, Southall UB1 3AF (Phone: +44 (0) 208 571 71 05) to act as my agent for the import, customs clearance, and handling of the deceased (body or ashes) upon their arrival into the United Kingdom.

I understand that the services provided by MAS Logistics (UK) Ltd will commence once the deceased has arrived at the UK airport.

These services include:

Liaising with the airline and airport handling agents.

Completing all necessary UK customs clearance procedures.

Liaising with Port Health authorities to obtain any required approvals for entry into the UK.

If required: Collecting the deceased from the airport and transferring them into the care of the chosen funeral director.

Client Responsibilities:

I confirm that all necessary preparations required by the exporting country and the airline, including embalming and the provision of a suitable repatriation coffin /urn, have been completed prior to the deceased's departure to the UK.

It is paramount that MAS Logistics (UK) Ltd can contact me (the applicant) at short notice via telephone or email to communicate urgent requests or information regarding the clearance process. I will ensure I am readily available until the deceased has been cleared and is in your care.

Should any delays in clearing the deceased from the airport or arranging onward transport be caused by either customs checks incorrect documentation supplied from the origin, or any other reason attributable to the applicant, MAS Logistics (UK) Ltd reserves the right to charge for any additional storage or administrative fees incurred.
